Home Care


Our care and services are provided wherever the patient calls home and are coordinated and supervised by a registered nurse or therapist. Planning for care includes identifying the patient’s goals and the potential obstacles he or she could face. Recovery from injury, surgery or illness and disease self-management begins with understanding and identifying symptoms, managing medications, and learning to deal with the anxiety, fear and feelings of isolation that may occur with illness.

The Home Care Team:
  • Provides comprehensive skilled nursing care including infusion/I.V. therapy
  • Educates patients, families (caregivers) on disease self-management and newly diagnosed diseases such as diabetes, Alzheimer’s, heart failure and chronic obstructive pulmonary disease.
  • Assesses new medications, changes in frequency and dosage as well as interactions with other medications.
  • Performs therapy services to help patients recovering after surgery, hospital or skilled nursing facility stays. For example: join replacement, stroke, or injury.
Our Support Team:
  • Coordinates with local clergy as requested
  • Coordinates additional community resources and support for patients and families to help alleviate feelings of loneliness and being overwhelmed, while encouraging personal choices.
Who pays for home care?
Home health care is usually covered under Medicare, Medicaid, and many health care plans including HMOs, PPOs, Managed Care and private insurance. Our administrative team is available to verify your benefits, coordinate coverage and complete the billing process.
Hospice Care
Hospice isn’t a place. It’s a philosophy of care. Heartland Hospice is dedicated to providing patients with care wherever they consider home. In the house they’ve owned for years. In a nursing home. Or in an assisted living facility. Hospice is caregiving tailored to fit the patient’s and family’s unique situation.

Our caregivers help manage pain and symptoms, enabling our patients to live their lives as fully and comfortably as possible. Social workers and chaplains create a comforting environment in which patients and families are able to share their thoughts, hopes and concerns. And special bereavement programs provide emotional support to patients and families who need help coping.

Who pays for hospice care?
Hospice is a 100% covered benefit under Medicare. This benefit covers the whole team's services, medications, equipment and supplies related to the patient's life-limiting illness. Hospice care is also covered under Medicaid and many insurance plans. Heartland Hospice provides care to those who qualify for hospice services regardless of their ability to pay, or insurance coverage.

Medicare’s hospice guidelines include both of the following:
  • Documented life-limiting prognosis by a certified physician(s)
  • Patient elects to receive comfort (palliative) care

I.V. Care
Home I.V. care, sometimes referred to as infusion care or infusion therapy, is the administration of I.V. medications or nutritional supplements through a catheter placed in the bloodstream or digestive system. Conditions that may require home I.V. care are: infections; cancer; Multiple Sclerosis; and diseases of the digestive tract.

Our home I.V. care services are provided by specially trained pharmacists and nurses who work with your physician to oversee the administration of I. V. mediations and tube feedings at home. Our I.V. pharmacies prepare medications, provide supplies and coordinate care with your physician and nursing provider. Our goal is to make I.V. care safe and easy in the home environment. At Heartland, we make home I.V. care simple and easy to access:
  • Medications mixed under sterile conditions into a ready-to-use form, usually a bag or syringe
  • Supplies and equipment for the infusion of the medication
  • Supplies to care for the catheter site
  • Delivery of medications and tube feeding material directly to the patient’s home
  • Consultations from pharmacists, usually at least once a week, to monitor side effects and verify supplies
  • Visits from nurses to your home to teach about I.V. care, and to change the catheter dressing and draw lab samples
  • Direct billing to the insurance provider
  • A customized plan of care developed by the pharmacist and nurse
  • Communication with the physician
