We have over 1,300 outpatient dialysis facilities and acute units I over 800 hospitals. We are located in 42 states and the District of Columbia, serving approximately 103,000 patients.

DaVita Titletown, on the west side of Green Bay, has highly qualified teammates that provide in-center hemodialysis 6 days per week. With over 40 years of experience, our staff of Registered Nurses and Patient Care Technicians utilizes the latest technology and techniques to ensure exceptional patient care. Our Masters prepared Renal Social Worker provides ongoing monitoring of psychosocial, financial, and emotional needs; along with assisting patients and families in coordinating various transportation and travel needs. Our Registered Renal Dietitian provides continuous dietary education with patients and families, as well as coordinating medication changes and evaluating laboratory results with the Bay Area Nephrologists. We take a team approach with monthly care planning to ensure continuity of care.

DaVita Northwoods is located on the northwest side of Shawano and provides in-center hemodialysis care 6 days per week for Shawano and the surrounding communities. Our facility is the dialysis center of choice for the vast hunters, fishermen, and vacationers that come to the area. Our social worker and dietitian provide on going monitoring education and support for established patients as well as those visiting. Registered Nurses and Patient Care Technicians provide quality care through advanced techniques and systems. A multidisciplinary team approach ensures the highest level of care and patient satisfaction. The physicians at Bay Area Nephrology, Fox Valley Nephrology, and Aurora Baycare Nephrology are supportive to both the dialysis facility and community in providing consultation services to local clinics.

DaVita Fox River was originally Royce C. Lin Memorial Dialysis Center, acquired by DaVita October 1, 2005. Our facility offers 3 modalities, Hemodialysis, Peritoneal and Home Hemo Dialysis (HHD). We are certified for 28 hemodialysis stations and are currently utilizing 22. The clinical staff is well trained and some have worked at this facility as many as 22 years. Our normal patient treatment hours are from 5:30 A.M.-9:00 P.M. Monday, Wednesday and Friday and 7:00 A.M.-4:00 P.M. Tuesday, Thursday and Saturday. Our facility was completely renovated in 2007, which included adding an elevator to give easy access to our Home Dialysis Clinic in the lower area. The lower area of the facility offers training for Peritoneal as well as Home Hemo Dialysis. Experienced RN staff will continue to monitor your care under the supervision of one of our nephrologists associated with our facility. We offer the Fresenius Baby K as well as the Nxstage system for home-dialysis. Nxstage allows the patient the flexibility in their life to be able to travel with the machine as it only weighs approximately 70 pounds. DaVita Fox River continually offers services and welcomes transient patients for all three modalities.
