Why does the patient have leg pain when the origin of the disease or injury occurs in the back?
The nerve from the leg goes through the back on the way to the brain, a herniated disc in the back pinches on the nerve, and the brain feels the nerve pain in the leg. The surgeon is able to localize the herniated disc in the back by the presentation of symptoms in which part of the leg.
Why should I seek treatment from a neurosurgeon over an orthopedic surgeon for my herniated disc?
Neurosurgeons are better suited for the delicate work around the nerves. They feel quite comfortable working on the brain. The spinal nerves are an extension of the brain itself. Neurosurgeons are more at ease and adept in manipulating the nerves in the spine.
Is surgery always necessary or are there alternative treatments?
Other that in an emergency, surgery is always the treatment of last resort. Patients are encouraged to try medication, to rest, limit activity levels, steroid injections, physical therapy, acupuncture and even chiropractic treatment, along with pain management prior to any thoughts of surgery. Debilitating symptoms not responsive to conservative care are then considered for surgery.
What types of pain management options are offered for patients with spine disease/ injuries?
Patients may be offered a multitude of pain management options such as medications. This may include steroids, nonsteroidal pain medication, non-narcotic pain medication as well as narcotic pain medication as needed. The patient may also have the option for physical therapy or acupuncture. For non-operative chronic pain they may be offered something like a morphine pump or spinal cord stimulators, which are implantable devices in the spine to control medication through electrical impulses or small does of pain medication.
What new surgical techniques are available for faster healing and less pain associated with traditional surgery?
The trend in surgery now is toward minimally invasive procedures, which require less surgical exposure, less trauma to tissue with faster healing and recovery times. Smaller instruments are used to cause less injury to the surgical site, at the same time offering sufficient exposure for the surgeon to complete the task. Patients are given the option of kyphoplasty for bone fracture repair. In this procedure, cement is placed into the fractured bone to stabilize it. This was not offered as recent as three years ago. Up until that point patients were treated with a brace and pain medication, and suffered significant pain for six months or more secondary to a spinal fracture.
