BestCare's experienced nurses and therapists coordinate and provide home health care prescribed by the patient's physician. Home Health visits reduce and prevent hospitalizations, and facilitate quicker discharges by providing immediate follow up by a clinician. Home health care facilitates improving compliance, stability and comfort for patients, families and caregivers in their own home.
Since 1980 BestCare Home Health Agency has provided medical services of Skilled Nursing Care, Physical Therapist, Occupational Therapist, Speech Therapist, Medical Social Worker and Home Health Aide services to homebound patients in his/her place of residence. We service four counties in the Central San Joaquin Valley of California: Fresno, Madera, Kings and Tulare. BestCare Home Health Agency is Medicare certified, CHAP accredited and is licensed in the State of California.
Skilled Nursing Services
Experienced Registered Nurses perform a variety of services prescribed by the patient's physician. BestCare's nursing staff assists the physician in determining the patient's treatment needs and performs ordered care and maintains contact with the physician. Our nursing staff is skilled in wound care, instruction and administration of medications, catheter care, pain management and diabetic management services.
Specialty Nursing Services
Enterostomal Therapy: Our Wound Care program is comprised of specially trained and certified wound care nurses who utilize advanced technology to offer the most comprehensive wound and ostomy care.
Telehealthcare: Enables our clinical staff to monitor, evaluate and educate acutely and chronically ill patients between regularly scheduled nurse visits. Clinical information is monitored every day while the patient stays where they belong, in their home.
Rehabilitation Program
A professional staff of licensed physical, occupational and speech therapists is available to assist the patient's rehabilitation needs.
Physical Therapy: Addresses mobility, strength and/or balance. Physical therapists will typically instruct the patient in home exercise program and safety issues.
Occupational Therapy: Focuses on the restoration of the functional aspects of daily living. This includes training to increase independence in dressing, bathing or other general household activities.
Speech/Language Pathology: Evaluates and treats speech, voice, swallowing and cognitive difficulties.
Medical Social Worker
As a service to patients receiving other skilled services, Social Workers assist the patient and family or caregivers with issues related to the recovery from illness or injury such as lifestyle changes and accessing community resources and counseling.
Certified Home Health Aides
Services provided by our Certified Home Health Aide are established by a Registered Nurse or Therapist to meet each patient's unique and specific needs. Service may include assistance with bathing, grooming, skin care, oral care, support with hygiene and sanitary matters, as well as limited meal preparation. This service is available only in conjunction with nursing or therapy services.
